Overview
Faircape Health is seeking a Locum Social Worker to provide psychosocial support services at their rehabilitation facility and retirement village locations in Somerset West and Tokai Estate. The role involves working alongside a multidisciplinary team to assist patients with complex medical needs and support their families throughout the rehabilitation journey.
Requirements
- Current SACSSP registration
- Professional indemnity insurance
- Minimum of 3 years' social work experience
- Experience in hospital social work and case management
- Training and experience in palliative care (essential)
- Strong report writing and time management skills
- Ability to work collaboratively within a multidisciplinary team
- Willingness to travel for outpatient consultations
- Experience in geriatrics, oncology and curatorship will be advantageous
Responsibilities
- Conduct psychosocial assessments and contribute to discharge planning
- Provide counselling, emotional support and mediation for patients and their families
- Coordinate multidisciplinary care and facilitate referrals to appropriate community and healthcare services
- Identify and address barriers to recovery, treatment adherence and successful discharge
- Maintain accurate clinical documentation and participate in multidisciplinary team discussions
- Advocate for patients' social, emotional and practical needs throughout their care journey
